About the Role

We are seeking a highly motivated and compassionate Care Team Leader to join our team on a 12-month fixed term contract. As a key member of our leadership team, you will play a vital role in ensuring the highest standards of care are delivered to our residents.

Responsibilities:
  • Lead and supervise your shift, providing guidance and support to care assistants as needed.
  • Case manage residents, working closely with families, healthcare professionals, and other stakeholders to develop personalized care plans.
  • Administer and record medication accurately and safely.
  • Conduct risk assessments to identify potential hazards and implement preventative measures.
  • Work collaboratively with care assistants to support residents with daily needs, promoting independence and activity.
  • Collaborate with the leadership team to develop services and drive continuous improvement.
Requirements:
  • NVQ Level 3 in health and social care or equivalent experience.
  • Excellent organizational and communication skills, with the ability to allocate and delegate work effectively.
  • Proven leadership and supervisory experience, with a strong track record of developing and motivating teams.
  • A good understanding of care planning processes and procedures.
  • Ability to administer medication and undertake training as required.
  • A caring and empathetic approach, with a passion for making a positive difference in the lives of others.
