Care Coordinator Specialist

We are working closely with a Local Authority in Enfield to assist with the appointment of a Care Coordinator, on a 3-month contract, likely to be extended at the client's discretion. Please apply with your CV for immediate consideration.
Rate of Pay: £16.30 - £19.93 per Hour
Responsibilities:
- To undertake initial assessments within the area of specialism.
- To manage, monitor and review care plans involving users and carers and liaising with other workers and agencies as appropriate.
- To work at all times within the council's Financial Regulations.
- To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the Group.
- To make use of the management information systems, including information technology, in the recording retrieval and analysis of information as required by the department.
Qualifications:
- As a regular and intrinsic part of this role requires you to speak to members of the public in English, the ability to converse at ease with customers and provide advice in accurate spoken English is essential and consistent with the requirements of this role. This role also requires you to be polite and courteous when conversing with the public.
- Ability to assess need and to make appropriate support plans and have a good understanding of the care management process.
- Ability to work in partnership with service users, carers and other professionals whilst maintaining the service user at the centre of each process or activity.
Please note:
- You should be available to work immediately or at a short notice.
- You should have the right to work in the U.K.
- This role requires a basic DBS.
