Adult Care Coordinator Opportunity

Sanctuary Personnel Ltd, a pioneering recruitment agency, is seeking an experienced Adult Care Coordinator to join their Integrated Learning Disability Team. This permanent position is based in Enfield and offers a competitive salary of up to £32,805 per annum.

The successful candidate will be based in a modern, open-plan office in Enfield Town, surrounded by amenities and transport links. The role involves working with service users, their families, and external partners to identify and meet their needs. The Care Coordinator will be responsible for assessing needs, developing support plans, and arranging and amending support to ensure the best possible outcomes.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work in partnership with service users, carers, and other professionals. They will also have a good understanding of the care management process and be able to work within the departmental financial policies and controls.

  • To provide assessment, care management, and social work services to users and carers arising out of the council's duties and powers under legislation.
  • To manage, monitor, and review care plans involving users and carers and liaise with other workers and agencies as appropriate.
  • To work at all times within the council's Financial Regulations.
  • To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the group.
  • Carry out accountabilities in compliance with the councils policies and procedures.
