About the Role

We are seeking an experienced Diabetes Specialist Nurse to lead our Housebound Diabetes Service. As a Clinical Nurse Specialist, you will be responsible for developing and sustaining relationships with individuals, groups, and agencies within a strategic framework.

Key Responsibilities
  • Lead in professional support, operational management of the housebound diabetic caseload, and clinical leadership, supporting peers, District Nursing Service, and locality leads.
  • Develop and sustain relationships and networks with groups, individuals, and agencies within a strategic framework.
  • Support peers and team leaders, and be an effective team member while working on your own initiative.
  • Act as a source of clinical knowledge and expertise to people with Diabetes, their carers, health, and other statutory and non-statutory agencies.
  • Work as part of the Community Diabetes Service, in delivering services as directed.

Requirements
  • NVQ level 2 or equivalent standard of literacy and numeracy.
  • Registered Nurse on Part 1 of the NMC.
  • ENB 998.
  • Post Registration qualification in Diabetes.
Desirable Qualifications
  • First Degree or equivalent experience.
  • Non-Medical Prescribing or working towards.
Knowledge and Skills
  • Awareness of NHS Plan, NSF, and clinical governance priorities.
  • Up-to-date knowledge of all aspects of diabetes care and broad experience of working within this specialty.
  • Understanding of multidisciplinary and inter-professional practice.
  • Experience of working in a community setting.
