We are seeking a highly skilled and dedicated Qualified Professional to join our Mental Health Team at Ackerman Pierce Ltd. As a key member of our team, you will be responsible for delivering high-quality care and support to vulnerable adults with mental health needs.

Your primary responsibilities will include:

  • Providing comprehensive assessments of clients' needs and circumstances, ensuring compliance with relevant legislation and policies
  • Developing and implementing personalized care plans that address clients' unique requirements and preferences
  • Liaising with other healthcare professionals, families, and carers to ensure seamless care delivery and coordination
  • Monitoring clients' progress, identifying areas for improvement, and adjusting care plans accordingly

To be successful in this role, you will need:

  • Post-qualifying experience of working within a Qualified Social Worker - Mental Health Team within the UK Social Care industry
  • Registration with the HCPC, as well as an Enhanced DBS/CRB certificate
  • At least 1 year of post-qualifying experience in the UK, with a strong understanding of relevant legislation and policies
  • Eligibility to work in the UK
