Key Responsibilities:- Conduct comprehensive Social Care needs assessments, identifying areas of concern and developing targeted interventions.
- Collaborate with the Pilot Front Door Team to achieve targets and provide high-quality support services.
- Develop and maintain accurate case records, ensuring transparency and accountability.
- Engage with service users, families, and other stakeholders to build trust and promote effective communication.
- Degree or equivalent in social work.
- Current Social Work England registration.
- Proven experience in case management and social work practices.
