About this role: We are seeking a compassionate and dedicated Health Support Professional to join our team at SE Health. As a Health Support Professional, you will play a vital role in providing high-quality care and support to our clients in their homes.



Key responsibilities:



  • Provide personal care and support to clients, including assistance with daily living activities, medication management, and mobility
  • Assist clients with household management, including meal preparation, cleaning, and laundry
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination
  • Participate in ongoing education and training to enhance skills and knowledge in healthcare
  • Maintain accurate records and reporting of client care and progress


Requirements:



  • High school diploma or equivalent required; post-secondary education in a related field an asset
  • Valid driver's license and reliable transportation required
  • First Aid and CPR certification required; level C an asset
  • Proof of updated immunizations and negative TB test required
  • Police and vulnerable sector check required
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
