- Enable service users to participate in and contribute to the local community.
- Establish and maintain professional relationships with service users, contributing to a homely and pleasant living environment.
- Conduct yourself in a professional manner, acting as a role model to colleagues and service users, and contributing to the delivery of quality services.
- Facilitate daily routines, including personal care, promoting high standards of personal hygiene.
- Participate in developing service user support plans, risk assessments, and programs, and complete written records as necessary.
- Implement individual support plans, highlighting any issues or concerns to the Team Leader to ensure quality services are delivered.
- Participate in individual service user review meetings and support plans, within boundaries set by senior colleagues.
- Enable service users to look after and clean their own rooms, organize their personal possessions, clothing, and other personal effects.
- Assist in cleaning and maintaining all rooms, fixtures, and fittings, maintaining a safe environment for service users and staff.
- Enable service users to access and utilize resources and activities available within their local and wider communities.
- Accompany service users as required, ensuring they get to and from activities safely and promptly.
- Act as a Keyworker for designated service users, liaising closely with relevant staff regarding individual programs.
- Report any enquiries, issues, or concerns to the line manager, providing information to enable them to take appropriate action.
- Liaise professionally with service users' family members, friends, advocates, and other professionals as required.
- Ensure all relevant documentation is completed accurately and effectively in a timely manner.
- Attend and contribute to annual reviews, multi-disciplinary meetings, and Person-Centred Planning meetings, and all other relevant meetings as required.
- Contribute to daily handovers between shifts, ensuring effective communication between staff, including all communication documentation.
- Act as a shift leader as required at the location.
- Support and administer medication following relevant training, in line with the support plan and individual guidelines.
- Support and facilitate financial transactions for service users and the Trust, in line with the individual support plan and company policies and procedures.
- Support service users' health and wellbeing, including attending appointments, hospital admissions, and promoting healthy lifestyles.
- Work in accordance with the rota system, including working unsociable hours at weekends and some bank holidays, and sleep-in duties may be required at specific locations.
- Undertake any reasonable travelling requirements to fulfill the tasks and responsibilities of the role.
- Support service users, if requested, on holidays away from the location.
- Undertake any other duties as may from time to time be specified by the Trust, within the level and responsibility appropriate to the grade of post.
- Ensure all essential and core training is undertaken and remains current, in line with requirements of the role.
- Participate in regular support and supervision from line management.
- Support service users who may present challenging behavior and complex needs, implementing approved strategies in accordance with relevant programs.
- Be aware of all service user support plans and program changes to assist self and colleagues in supporting service users as required.
- Continuously develop professional knowledge and expertise, attending and participating in learning and development events, meetings, conferences, and workshops as requested/approved by line management.
- Attend and participate in regular team and other meetings, both internal and external.
- May be required to carry out training in addition to this substantive post.
- Be familiar and comply with the local Code of Conduct, relevant to the location.
- Be familiar and comply with written policies, procedures, and guidelines for good practice, issued by Brainkind.
- Ensure the Health & Safety policy and procedures are adhered to at all times.
