Palliative Care Triage Nurse Role

Royal Trinity Hospice is seeking an experienced Palliative Care Triage Nurse to join our First Contact team. As a key member of our clinical team, you will play a vital role in receiving and managing referrals made to the hospice, ensuring timely and effective care for patients.

Key Responsibilities
  • Receive and manage referrals, prioritizing patient care needs and determining the appropriate level of care required.
  • Utilize agreed resources and guidelines to assess, advise, and schedule patient care management or refer patients to other health and social care agencies when necessary.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, ensuring care is planned to meet individualized and dynamic needs.
  • Train, coach, and mentor clinical staff and visitors to the hospice about specialist palliative care practice.
Requirements
  • Registered Nurse with bachelor's degree level training in palliative/end of life care or related subject and extensive experience working within a specialist palliative care field.
  • Advanced assessment skills, critical thinking, and communication skills.
  • Able to demonstrate knowledge of disease trajectories and treatment effects.
  • Ability to work autonomously and as part of a team, with a flexible attitude to working.
