Vulnerable Population Support Specialist

This senior practitioner role is an exciting opportunity for a dynamic and committed member of staff to work in a multi-disciplinary environment, supporting vulnerable individuals and their families within hospital and community settings. As part of the Integrated Triage and Assessment team, Porth Gofal Service, you will work closely with local authorities and healthcare providers to ensure seamless coordination and effective support for citizens.
About the Position
This role requires strong leadership skills, a commitment to serving the community, and the ability to work effectively in a bilingual environment. You will be responsible for supporting citizens living in Ceredigion by applying a strength-based needs assessment approach, focusing on service user choice and control using the Signs of Safety framework.
Duties and Responsibilities
The key responsibilities of this role include:
- Providing information, advice, and assistance about community health and social care services for adults.
- Supporting more consistent and coordinated assessments for care and support.
- Improving the flow of information between services to facilitate faster decision-making about people's needs and support.
- Rapid wrap-around reablement response services to reduce hospital admissions.
- In-reach services to reduce delayed transfers of care by providing timely discharges from hospital.
Requirements
To be considered for this role, applicants should have a strong background in social work, excellent communication skills, and the ability to work effectively in a team environment. A willingness to learn Welsh and achieve the ALTE standard within two years is also essential.
