Main Responsibilities
  • Provide comprehensive specialist assessment, review, and evaluation of patient needs and develop personalized care plans to promote health gain and maximize independence.
  • Collaborate with a multidisciplinary team to ensure timely and effective delivery of care, allocating workloads and coordinating patient journeys.
  • Develop and maintain partnership working with various professionals to ensure seamless care delivery.
  • Manage complex cases, demonstrating expertise in leadership, care planning, and care delivery.
  • Support the development of clinical pathways and contribute to the improvement of patient outcomes.

Requirements
  • Registered Allied Healthcare Professional with HCPC registration.
  • Degree or Diploma in Physiotherapy or Occupational Therapy.
  • Experience of recent Continuous Professional Development.
  • Valid and current driving license and car for travel throughout Buckinghamshire.
Desirable Criteria
  • Relevant postgraduate clinical training in areas such as Elderly Long Term Conditions, Respiratory Disease, Cardiac Disease, Trauma, Orthopaedics, and End of Life.
  • Experience of working in a community or in-patient setting for a minimum of 2 years post-registration.
  • Experience of working within a multi-agency/multi-disciplinary team.
  • Experience of working with single assessment processes and electronic clinical systems.
Skills and Abilities
  • Demonstrate profession-specific clinical assessment, planning, delivery, and evaluation skills.
  • Ability to manage complex cases and develop partnership working.
  • Knowledge of legal requirements for working with vulnerable adults and those who lack capacity.
  • Ability to motivate staff and contribute to a positive work environment.
  • Excellent written and verbal communication skills, with the ability to report writing and present information effectively.
