The estimated salary for this role is £40,000 - £55,000 per annum, depending on experience.

Job DescriptionKey Responsibilities:
- Support families in preparation for transition to adult services
- Work alongside teams to ensure smooth transition
- Liaise with multi-agencies to ensure wrap-around support for young people
- Develop services to meet the needs of young people transitioning to adult services
- Provide training and education to staff and stakeholders on the needs of young people moving from CAMHS to adult services
