Job Summary

An exciting opportunity has arisen for a highly motivated and passionate clinical practitioner to join our team working to prevent unnecessary admissions to mental health hospitals of adults with a learning disability and/or autism.

As a clinical practitioner, you will manage a caseload of individuals who have been referred to the Dynamic Support Pathway and have been identified as having complex needs requiring additional support to coordinate and manage their care.

You will be experienced in working independently and as a member of a multi-disciplinary team, with the ability to work across numerous agencies while keeping the individual at the centre of everything you do.

You will have excellent communication and interpersonal skills, be able to problem-solve, and demonstrate the Trust Values in everything you do.

Whilst the role primarily works with adults, the Dynamic Support Pathway is an all-age service, and experience of working with children and young people is desirable.

You will join the award-winning LDA Collaborative and be part of making a difference and improving the lives of local people.

Main Duties of the Job

  • To work autonomously, taking responsibility for care, quality, and clinical activity for a designated caseload of service users.
  • To contribute to care quality performance and clinical activity by providing professional leadership and guidance, clinical advice, and management in relation to the key dimensions of healthcare quality.
  • Responsible for the assessment, planning, implementation, and evaluation of programmes of evidenced-based care to a group of service users according to changing healthcare needs and varying levels of complexity.
  • Work within a multi-disciplinary team to ensure effective care plans and treatments are in place for complex service users.
  • Recognise, prioritise, and respond appropriately to urgent and emergency situations.
  • Work collaboratively and ensure maintenance of effective partnership communication and with other professionals, agencies, and stakeholders to ensure needs are met in relation to care input, support for ongoing care needs, and safe discharge.
  • Ensure that fully accurate, complete, and contemporaneous clinical records are written and maintained for each service user in accordance with agreed policy.
  • Actively participate in the development and implementation and delivery of new ways of working, implementing research-based practice in order to promote ongoing care improvement.
