About the Role

We are seeking an exceptional Community Diabetes Service Lead to manage our community-based specialist diabetes service. As a key member of our team, you will be responsible for delivering high-quality care to adults living with diabetes.

Key Responsibilities
  • To provide clinical leadership and take responsibility for the provision of high-quality care in the service to the local community in accordance with departmental guidelines.
  • To lead on specific professional initiatives in collaboration with other Diabetes team members.
  • To be responsible for service development, particularly to develop integrated working and to develop improved patient pathways across primary / secondary care services.
Requirements
  • Highly experienced in managing complex diabetes patients in a variety of settings.
  • Able to make autonomous, evidence-based decisions and lead by example to educate and support colleagues in primary and secondary care.
  • Responsible for maintaining service standards and for all documentation to meet all legal requirements.
