Job Summary: We are seeking a highly motivated and empathetic individual to join our team as a Post Diagnostic Support Link Worker in Fife locality.

About the Role: As a Post Diagnostic Support Link Worker, you will provide high-quality, person-centred support to people living with dementia and their families for a minimum of a year following diagnosis.

Key Responsibilities:

  • Provide emotional support and guidance to individuals and their families affected by dementia.
  • Work closely with the Community Mental Health Team at Queen Margaret Hospital to deliver a comprehensive support service.
  • Develop and maintain relationships with colleagues, stakeholders, and community partners to ensure effective service delivery.
  • Utilize digital platforms to facilitate virtual meetings and online groups, promoting a blended approach to support.

Requirements:

  • Recognised relevant professional qualification.
  • Good understanding of dementia and its impact on individuals and families.
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly and empathetically with individuals and families.
  • Enhanced Level of the Promoting Excellence Framework or working towards completion.
