Child and Family Care Specialist

We are seeking a skilled Clinical Support Coordinator to join our team at Coordinated Care Services, Inc. The successful candidate will be responsible for providing psychosocial assistance to patients aged 18 years and older, ensuring continuity of care upon discharge.
Key Responsibilities:- Conduct telephone screenings on new referrals to determine suitability for clinic services, and assign patients to the most appropriate intake slot.
- Assist the clinical team with patient and family follow-up, including outreach, scheduling, and collaboration with current providers and social supports.
- Assess the needs of patients and complete referrals for services such as transportation, care management, and other essential support.
- Provide social work follow-up to patients, families, and agencies after discharge to ensure continuity of care and integrity of the discharge plan.
- Bachelor's Degree in Social Work from an accredited program.
- Ability to communicate effectively with a diverse group of medical providers.
The estimated salary range for this position is $43,000 - $53,000 per year, based on experience and qualifications. We offer a comprehensive benefits package, including health insurance, retirement plans, and paid time off.
