Role Overview

We are seeking a skilled Advanced Cardiac Rehabilitation Specialist to join our team at Chelsea and Westminster Hospital NHS Foundation Trust.

This is an exciting opportunity for a heart failure specialist nurse to contribute to the delivery of high-quality care in cardiac rehabilitation, working closely with a multidisciplinary team to achieve best practice outcomes.

The successful applicant will have extensive experience in cardiology and a proven track record of developing and implementing patient-centered care pathways. They will be responsible for managing their own caseload, including nurse-led clinics, virtual consultations, and education programs for patients with heart failure.

A key aspect of this role is to work collaboratively across both Chelsea and Westminster Hospital and West Middlesex University Hospital sites, contributing to the development of heart failure services through audits, quality improvement initiatives, and strategic planning.
