Mental Health Practitioner

This is an exciting opportunity for a dedicated healthcare professional to join our team as a Community Mental Health Practitioner. We are seeking an individual with a core profession of Occupational Therapy, Speech and Language, Mental Health Nursing, or Social Work to promote and improve the mental health and well-being of young people and those who care for them.
Main Responsibilities- Assess and manage risk, including safeguarding, and provide evidenced-based assessments, formulations, and care plans.
- Work collaboratively with children, young people, and their families to provide therapeutic solutions in line with NICE guidelines.
- Manage a defined caseload and have the responsibility for the assessment, treatment, and discharge of young people.
- Ensure effective and sensitive communication between children, young people, their families, and healthcare professionals.

Requirements- Proven experience of working effectively with people with mental health difficulties.
- Experience of assessing and managing risk, including safeguarding.
- Experience of providing evidenced-based assessments, formulations, and care plans.
- Experience of liaison and working with a range of other relevant agencies and professionals.
