We are seeking a skilled and compassionate Cardiac Rehabilitation Specialist to join our award-winning team at Kent Community Health NHS Foundation Trust.

About the Role

This is an exciting opportunity to work with patients who have experienced a cardiac event, providing them with support and guidance to aid their recovery. As a key member of our multi-disciplinary team, you will play a vital role in helping patients manage their physical and mental well-being.

The successful candidate will possess a good understanding of heart disease and its associated risks. Additionally, they will have experience working with vulnerable individuals and be able to drive with access to a vehicle. The ideal candidate will be a strong communicator with excellent interpersonal skills.

Key Responsibilities:

  • Work closely with patients to develop personalized care plans
  • Provide education and support on heart disease management and risk reduction strategies
  • Collaborate with other healthcare professionals to ensure seamless patient care

Salary: £25,500 - £31,000 per annum, depending on experience. We offer a comprehensive benefits package, including pension scheme, life assurance, and generous annual leave.
