Mental Health Nurse

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our team at Langworth Ward, Witham Court. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with mental health needs associated with dementia.
Key Responsibilities- Develop and implement individualized care plans in collaboration with patients, carers, and other healthcare professionals.
- Provide expert nursing care to patients with complex mental health needs, including those with dementia.
- Conduct comprehensive assessments and risk assessments to identify patient needs and develop effective care plans.
- Work closely with the Dementia Home Treatment Team to ensure seamless transitions and effective discharge planning.
- Participate in multidisciplinary team meetings and contribute to the development of care pathways and service improvements.
- Supervise and mentor junior staff and students to ensure high-quality care and professional development.
- Registered Mental Health Nurse (RMN) or equivalent qualification.
- Experience working with older adults with dementia and complex mental health needs.
- Sound knowledge of clinical risk assessment and information governance principles.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- Ability to travel independently across the county and work flexible shifts, including evenings, nights, and weekends.
