About the Role

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our team at Langworth ward, Witham Court. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with mental health needs associated with dementia.

Key Responsibilities
  • Develop and implement individualized care plans in collaboration with patients, carers, and other healthcare professionals.
  • Provide expert nursing care to patients with complex mental health needs, including those with dementia.
  • Conduct comprehensive assessments and risk assessments to identify patients' needs and develop strategies to mitigate risks.
  • Work closely with the Dementia Home Treatment Team to ensure seamless transitions between hospital and community care.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve outcomes.
  • Supervise and mentor junior staff and students to ensure high-quality care and professional development.
Requirements
  • Registered Mental Health Nurse (RMN) or equivalent qualification.
  • Experience working with patients with dementia and complex mental health needs.
  • Sound knowledge of the national agenda for mental health and dementia care.
  • Ability to work independently and as part of a team to deliver high-quality care.
  • Excellent communication and interpersonal skills.
