We are seeking a skilled and compassionate Social Prescriber Link Worker to join our team. As a key member of our Multidisciplinary Team, you will play a vital role in providing personalized support to individuals, their families, and carers.
Key Responsibilities- Take referrals from a range of staff within the Multidisciplinary Team and provide tailored support to individuals based on their unique needs and priorities.
- Work collaboratively with individuals to co-produce a personalized care and support plan that focuses on improving health and wellbeing, introducing or reconnecting them to community groups and statutory services.
- Manage and prioritize a caseload, ensuring that individuals receive timely and effective support.
- Identify local community assets and support them to receive social prescribing referrals, ensuring they are open and welcoming to individuals.
- Contribute to developing new groups and networks to fill gaps in local community provision.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work collaboratively with individuals and community groups. You will also need to be able to manage a caseload effectively and prioritize tasks to meet deadlines.
