This is an exciting opportunity for a Nurse or Allied Health Professional to work as a Dementia Practice Educator (DPE) alongside a small Dementia Improvement Team across the Betsi Cadwaladr University Health Board.
Main ResponsibilitiesThe post holder will identify and support care home residents before admission or early after admission to prevent deconditioning and contractures. They will also plan and deliver learning opportunities for staff across care homes and secondary care settings to share knowledge of prevention of deconditioning/contractures/effective positioning.
About the RoleThe successful applicant will have excellent organisational and time management skills and be able to work flexibly to meet the needs of this new post. They will also have a passion for improving dementia care services and pathways.
RequirementsThe post holder will need to meet all essential criteria as per the attached job description/person specification. A passion for improving dementia care services and pathways is essential, as is significant and current experience of working within dementia care.
