About the Role

We are seeking a highly skilled and experienced Dementia Practice Educator to join our small Dementia Improvement Team at Betsi Cadwaladr University Health Board. The successful candidate will work alongside our team to prevent deconditioning and contractures amongst residents living with dementia in a group of selected care homes.

Main Responsibilities

The post holder will identify and support care home residents before admission or early after admission to develop an individualised deconditioning support plan. They will also plan and deliver effective learning opportunities for staff across care home and secondary care settings to share knowledge of prevention of deconditioning/contractures/effective positioning.

Requirements

The successful applicant will have excellent organisational and time management skills, and be able to work flexibly to meet the needs of this new post. Significant and current experience of working within dementia care is essential, as is a passion for improving dementia care services and pathways.
