About the Role

We are seeking a dedicated and compassionate Care and Support Assistant to join our team at Tilmore Gardens, a CQC Registered Learning Disabilities Service in Petersfield.

As a Care and Support Assistant, you will play a vital role in supporting our customers with learning and physical disabilities to maximise their potential and independence.

Key Responsibilities
  • Provide support to encourage customers to develop their skills and confidence
  • Key work with individuals to develop and implement care plans
  • Follow care plans and maintain accurate records
  • Deliver high-quality, personalised care and support services, including personal care and medication management
  • Support customers to engage in activities and outings, such as visits to local shops and cafes
  • Promote a positive culture by engaging with customers and preventing social isolation
  • Work collaboratively with colleagues to maintain a clean and comfortable environment
  • Identify and mitigate risks to ensure a safe and supportive environment
Requirements
  • Prior experience in delivering personal care is desirable
  • A kind and caring nature, with excellent communication skills
  • Ability to work effectively as part of a team and on own initiative
  • Flexibility and adaptability to meet the changing needs of customers
  • Excellent organisational skills and attention to detail
  • NVQ 2 in Health and Social Care, or willingness to work towards, is desirable
