Homelessness Inclusion Health Nurse Specialist

We are seeking a highly skilled and compassionate Homelessness Inclusion Health Nurse to join our team in Ealing. As a key member of our outreach team, you will be responsible for providing high-quality healthcare services to individuals experiencing homelessness.
Main Responsibilities:- Develop therapeutic relationships with individuals experiencing homelessness, using trauma-informed care principles to enable relationship building, assessment, and treatment.
- Undertake specialist homeless healthcare at an advanced level, including physical and psychosocial health assessments, and identification of unmet health needs.
- Act as a health case manager for individuals with high levels of unmet health need, working towards individual health outcomes as defined by project goals.
- Proactively identify immediate clinical risks to life and limb, and take appropriate action to enable individuals to access timely care.
- Independently undertake mental capacity assessments, and make best judgment decisions on whether individuals have mental capacity to refuse care.
- Provide immediately necessary clinical/therapeutic primary care treatments on the street, as needed.
- Ensure good communication of treatments offered, treatment progress, and outcome of investigations to other healthcare agencies.
- Proactively identify when individuals are not in contact with relevant primary healthcare services, and advocate for and support individuals to access these services.
- Identify ongoing safeguarding concerns, refer to safeguarding appropriately, and contribute to developing safeguarding plans.
- Develop and maintain close working relationships with homeless and inclusion health services, GPs, dentists, ambulance services, mental health services, addictions services, district nurses, community matrons, and acute trusts.
- Provide screening for blood-borne viruses or diabetes, as appropriate.
- Deliver vaccinations on the street, when indicated and safe to do so.
- Contribute to CGL Naloxone Strategy by training staff and individuals in the use of naloxone.
- Act as an advocate for individuals experiencing homelessness, supporting their own choices regarding care.
- Engage with training and clinical networks, as required by the manager.
- Experience of working as a clinical practitioner in an inclusion health setting.
- Broad knowledge and experience covering primary care, acute, mental health, and addictions.
- Ability to independently represent the service at local and national level.
- Experience of directly delivering nurse-led health street outreach to people who are rough sleeping.
- Understanding of how nurse-led health street outreach fits into the wider infrastructure of homeless outreach, housing, and social care, primary and emergency care, mental health, and addictions.
- Good understanding of legal literacy in respect to housing, immigration, and healthcare access.
- Good understanding of clinical governance issues, specifically in reference to health street outreach.
- Experience of independently managing projects.
- Experience of leading audit/quality improvement projects.
- Experience of report writing.
- Trained in phlebotomy.
- Qualification in substance misuse.
