About the Role:

Active Care Group is seeking a highly motivated and compassionate Rehabilitation Support Assistant to join our team. The ideal candidate will be responsible for providing support to a client with a brain injury, promoting their independence and quality of life. This role involves assisting the client with day-to-day care, rehabilitation, and community-based activities. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with the client, their family, and other healthcare professionals.

Key Responsibilities:
  • Assist the client with personal care, including bathing, dressing, and mobility support
  • Support the client with meal preparation, cleaning, and household tasks
  • Accompany the client to community events, social activities, and medical appointments
  • Assist the client in maintaining a safe and healthy living environment
  • Document the client's progress, needs, and preferences accurately and confidentially
About You:

The successful candidate will be a reliable, flexible, and compassionate individual with a passion for supporting individuals with brain injuries. You will possess excellent communication and interpersonal skills, with the ability to work effectively with the client, their family, and other healthcare professionals.
