Hampshire County Council Discharge to Assess Team Member

We are seeking a skilled Social Worker to join our team in a remote working capacity. As a member of our discharge to assess team, you will be responsible for conducting face-to-face assessments in various settings, including people's own homes and nursing/care homes.
This role requires the ability to complete care assessments with a strong understanding of Continuing Healthcare. You will work closely with other agencies to identify and address the needs of individuals, ensuring that care packages/services are tailored to meet their specific requirements.
The successful candidate will have a proven track record of delivering high-quality care services, with excellent communication and interpersonal skills. You will be able to work independently, with minimal supervision, and demonstrate a commitment to ongoing professional development.
Benefits
We offer a competitive salary of £32 per hour, paid on an umbrella basis. Other benefits include a hybrid tablet device, personal assistant support, 25 days annual leave, 5 personal development days and access to permanent vacancies as they become available.
