Advanced Nurse Practitioner for Care Homes and Community Settings

Livewell Southwest is seeking a highly skilled and experienced nurse leader to join our team as a Modern Matron for CAMHS Inpatient Care and Acute Pathway. As a key member of our leadership team, you will be responsible for providing professional leadership to the CAMHS Tier 4 inpatient unit, Children's Young People's Place of Safety, and the Acute Care Pathway.
Main Responsibilities- Provide line management and support to unit/ward/team managers as appropriate
- Work with South West Provider Collaborative Case Managers, Contract leads, and quality leads to ensure care delivered is in line with Tier 4 specification and access and referrals process
- Lead the Acute Care Pathway to ensure it works collaboratively with referral and admission and discharge process to and from Tier 4 inpatient facilities

- Educated at Degree/Masters level or equivalent senior level experience
- Current NMC registration in a field that would give significant exposure to children and young people who require mental health support
- Evidence of recent continuing personal learning/professional development
- RMN or RN Child
- Knowledge of current NHS policies/processes and structures, especially relating to acute inpatient care
- Detailed knowledge of CPA and standards of record keeping
- Ability to prioritise own workload and work on own initiative as an effective team member
- Excellent verbal and written skills, good numerical skills, and excellent facilitation and arbitration skills
- Experience of implementing change within a team and demonstrating outcomes achieved following implementation of service development
- Experience of working with recovery-focused models of care, which are based on least restrictive principles
- Experience of working in the CAMHS acute pathway
- Ability to travel across Plymouth and experience of working with care pathways
- Experience of data collection, analysis, and synthesis, and experience of using Microsoft Excel, Access, PowerPoint, and electronic mail
- Effective people management skills with ability to inspire, create positive relationships, and provide support to all team members
- Current driving licence and experience in project work/management
