Health Visitor Band 5

Croydon Health Services NHS Trust is seeking a highly skilled and dedicated Health Visitor to join our team. As a Health Visitor, you will play a vital role in delivering the Healthy Child Programme, working closely with families, children, and young people to promote their health and wellbeing.
The successful candidate will have a strong clinical background, excellent communication skills, and the ability to work effectively in a multidisciplinary team. You will be responsible for assessing client needs, planning and delivering evidence-based care, and working in partnership with other agencies to ensure the best possible outcomes for children and families.

Key Responsibilities- Deliver the Healthy Child Programme to families, children, and young people
- Assess client needs and plan and deliver evidence-based care
- Work in partnership with other agencies to ensure the best possible outcomes for children and families
- Develop and maintain effective relationships with families, children, and young people
- Contribute to the development of innovative clinical practice
- Registered General Nurse (RGN) qualification
- Specialist Public Health Practitioner Health Visiting qualification at Degree or Postgraduate Level
- Nurse Prescribing qualification or commitment to undertake Nurse Prescribing training
- Mentoring qualification
- Experience of implementing evidence-based practice
- Line management experience and community setting
