About this roleWe're seeking a highly skilled and empathetic individual to join our Multidisciplinary team. In this role, you'll assess and understand the personal care and support needs of patients, plan and provide person-centred care based on individual need, and maintain high standards of evidence-based practice.
Key responsibilities- Conduct thorough assessments to identify patient needs and preferences.
- Develop and implement personalized care plans that meet the unique needs of each patient.
- Provide gentle support with mobility, maintain personal hygiene, monitor vital signs, and create a calming environment.
- NVQ Level 3 in Health and Social Care OR an equivalent alternative Level 3 Qualification.
- Experience working within a Multidisciplinary team, assisting in curating individual assessments and advanced care plans.
- Flexibility to provide care in areas with the most need.
- Competitive salary of £20,000 per annum, commensurate with industry standards for a Senior Healthcare Assistant in the UK.
- 27 days annual leave allowance plus 8 public holidays (pro rata).
