About the Role

We are seeking a highly skilled and experienced Community Care Lead to join our team at Torbay and South Devon NHS Foundation Trust. As a key member of our leadership team, you will be responsible for providing strategic direction and leadership to our Intermediate Care team, ensuring the delivery of high-quality services that meet the needs of our patients and their families.

The successful candidate will have a strong background in healthcare leadership, with experience of managing complex care pathways and developing innovative solutions to improve patient outcomes. You will be a collaborative leader, able to work effectively with multidisciplinary teams, including healthcare professionals, social care workers, and voluntary sector partners.

Key responsibilities will include:

  • Providing leadership and guidance to the Intermediate Care team, ensuring the delivery of high-quality services that meet the needs of patients and their families.
  • Developing and implementing strategic plans to improve patient outcomes and reduce hospital admissions.
  • Working closely with other healthcare professionals and stakeholders to ensure seamless care pathways and effective communication.
  • Managing budgets and resources to ensure the efficient delivery of services.
  • Providing training and development opportunities to staff to enhance their skills and knowledge.
