About the Role

NHS Sussex ICB is seeking a highly skilled and experienced Senior Practitioner to join our Clinical Bank team. As a Senior Practitioner, you will play a key role in delivering an effective, efficient, and equitable service to the people and populations of Sussex.

Key Responsibilities

  • Assessment and review of patients to determine their eligibility for Continuing Healthcare
  • Development and implementation of care plans to meet the needs of patients
  • Case management and coordination of care services
  • Collaboration with multidisciplinary teams to ensure seamless care delivery

Requirements

  • Experience of working in a health or care environment, including acute, community, mental health, and local authority settings
  • Good understanding of the health and social care environment and roles and responsibilities within it
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
