Job Title: Care and Wellbeing Coordinator


Job Summary:


Benfleet Primary Care Network is seeking a skilled Care and Wellbeing Coordinator to join our team. This role is pivotal in ensuring that all patients receive the best possible care and service.


Key Responsibilities:



  • Work closely with our GP practices and the multidisciplinary team within the PCN to provide high-quality care and services.
  • Provide cross-cover to other PCN care coordinators during times of annual leave and sickness.
  • Support the delivery of training to PCN staff and assist with data quality issues related to PCN contracts, enhanced services, and IIF/QoF.
  • Collaborate with our team to develop and implement effective care plans that meet the needs of our patients.
  • Utilize strong administration and organisational skills to manage multiple tasks and priorities.
  • Work effectively as part of a multi-disciplinary team and take ownership of tasks and projects.

Benfleet PCN was formed in 2019 with seven like-minded practices working together to provide a joined-up, integrated service for our population. We serve just under 48,000 patients across our patch.
