We are seeking an experienced and skilled Community Matron to join our team at Epsom and St Helier University Hospitals NHS Trust. As a Community Matron, you will play a key role in delivering high-quality, patient-centered care to individuals with complex needs.
Key Responsibilities- Provide clinical triage and assessment to inbound referrals, assigning the appropriate urgency and determining the best clinician to undertake the visit from the multi-disciplinary team.
- Support annual health checks for individuals with Learning Disability or Serious Mental Illness.
- Coordinate care and provide proactive support to patients with complex needs, including those with cancer, cardiovascular disease, and hypertension.
- Work collaboratively with primary, secondary, and community teams to reduce hospital admissions and improve patient outcomes.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare professionals.
- Lead and implement the Clinical Governance Strategy within the practice area, facilitating clinical audit and monitoring of care.
- Lead and implement the Risk Management Strategy within the practice area, ensuring adherence to processes.
- Professional registration as a nurse or allied health professional.
- Previous experience in a community setting, preferably in a similar role.
- Understanding of proactive care processes and ability to work in a multidisciplinary team environment.
