Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, offering them hope, kindness, and a brighter future.

Key Responsibilities:
  • Provide clinical interventions and healthcare services to individuals in need, tailored to their unique needs and circumstances.
  • Work collaboratively with our outreach team to deliver innovative healthcare solutions and support services.
  • Embodiment of empathy, understanding, and unwavering support, serving as a lifeline for individuals facing the challenges of homelessness.
  • Implement evidence-based clinical interventions, ensuring that every individual receives the care and support they deserve.
Requirements:
  • Experience in providing clinical services to vulnerable populations, particularly those experiencing homelessness.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport with individuals from diverse backgrounds.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality services and outcomes.
  • Compassion, empathy, and a commitment to making a positive difference in the lives of others.
What We Offer:
  • A competitive salary range of £43,814.50 - £48,183.48 (pro rata for part-time hours), plus Inner London Weighting of £4,072.06 (pro rata for part-time hours).
  • A fixed-term contract until February 2026, with opportunities for professional growth and development.
  • A supportive and collaborative work environment, with a focus on delivering high-quality services and outcomes.
