Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, addressing their physical and mental health needs.

About the Role:
  • Provide healthcare and clinical interventions to homeless individuals, including assessments, treatments, and referrals.
  • Work collaboratively with outreach teams to identify and address the health needs of service users.
  • Develop and implement innovative clinical interventions to improve health outcomes for homeless individuals.
  • Contribute to the development and implementation of outreach services, ensuring they meet the needs of service users.
  • Collaborate with other healthcare professionals and agencies to ensure seamless care coordination.
