About the Role

We are seeking a highly skilled and experienced Advanced Fostering Practitioner to join our Community Hospitals and Frailty team. As a key member of our multidisciplinary team, you will be responsible for providing expert holistic clinical care to patients with complex needs.

Working in a fast-paced environment, you will be required to assess patients, manage ongoing and acute medical problems, and guide and advise the team. You will also be expected to work collaboratively with community partners, such as community beds, integrated care communities, and reablement services.

The successful candidate will have a strong background in frailty care, with experience in comprehensive geriatric assessment and undifferentiated diagnosis. You will also have excellent communication and interpersonal skills, with the ability to work effectively in a team environment.

Key Responsibilities
  • Provide expert holistic clinical care to patients with complex needs
  • Assess patients and manage ongoing and acute medical problems
  • Guide and advise the team on patient care
  • Work collaboratively with community partners to provide seamless care
  • Develop and implement care plans to meet individual patient needs
Requirements
  • Registered healthcare practitioner with substantial post-education experience in frailty care
  • Training and mentorship qualification or equivalent
  • Non-medical Prescribing V300 or willingness to work towards it
  • Clinical Educators course
  • Advanced Life Support
  • Leadership qualification or equivalent
