As a passionate and experienced professional, you will work with children and young people who have a range of disabilities, helping them and their families access the support they need to live fulfilling lives.
Key Responsibilities:
• Conduct holistic assessments of children with disabilities and their families, developing person-centered care and support plans to meet their individual needs.
• Advocate for children and young people, ensuring they receive the appropriate services and that their rights and best interests are upheld.
• Collaborate with schools, healthcare professionals, and other agencies to coordinate and deliver integrated care and support services.
• Identify and respond to safeguarding concerns, ensuring appropriate protective measures are taken in line with Cardiff's safeguarding policies and procedures.
• Provide advice, guidance, and emotional support to families, empowering them to care for their children and access resources.
• Respond promptly to crises, providing practical and emotional support to prevent family breakdowns and ensure child safety.
• Regularly review care and support plans to ensure they remain relevant and continue to meet the needs of the child and their family.
About You:
• A degree in Social Work (or equivalent) and current registration with Social Care Wales are essential qualifications for this role.
• Previous experience working with children with disabilities or within a similar setting is also required.
