About the Role

We are seeking a highly skilled and compassionate Home Health Care Professional to join our team at Quality Talent Group. As a Skilled Nursing Specialist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Develop and Implement Care Plans: Work with patients and their families to create personalized care plans that meet their unique needs and goals.
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Coordinate Care Services: Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document Patient Progress: Accurately and thoroughly document patient progress, including assessments, interventions, and outcomes.
Requirements
  • Current RN License: Must have an active and unencumbered RN license in the state of practice.
  • CPR Certification: Current CPR certification from the American Red Cross or American Heart Association.
  • TB Skin Test: Must have a current TB skin test within the last 12 months.
  • Reliable Transportation: Must have reliable transportation to travel to patient homes.
Preferred Qualifications
  • Home Health Experience: Previous experience in home health care is preferred but not required.
  • Strong Communication Skills: Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
