Job Summary

Sherwood Forest Hospitals NHS Foundation Trust is seeking a dedicated and proactive discharge specialist practitioner to play a pivotal role in ensuring the timely and effective discharge of patients across the Trust. The successful candidate will work closely with ward-based multi-disciplinary teams and the discharge hub to ensure the provision of specialist advice, develop and implement policies to manage the discharge process, and conduct Reason to Reside checks for all patients.

Main Responsibilities

The successful candidate will be responsible for ensuring all patients with long lengths of stay over 14 days have clear discharge plans, participating in the Length of Stay (LOS) report weekly meetings and other relevant meetings, and working 9-5 for 5 days over a seven day period.

Person Specification

The successful candidate will have excellent communication and time management skills, the ability to change, manage and motivate others, and knowledge of audit and research. They will also have a first level registered nurse or AHP qualification, a first degree, and a teaching qualification. Desirable qualifications include a Masters degree, community experience, and experience in HCOP.
