Job Summary

Sherwood Forest Hospitals NHS Foundation Trust is seeking a dedicated and proactive discharge specialist practitioner to join our team, focusing on discharge coordination. The successful candidate will play a pivotal role in ensuring the timely and effective discharge of patients across the Trust.

Main Responsibilities
  • Provide specialist advice to ensure the provision of high-quality care.
  • Develop and implement policies to manage the discharge process, ensuring seamless transitions for patients.
  • Ensure all patients with long lengths of stay over 14 days have clear discharge plans in place.
  • Conduct Reason to Reside checks for all patients to ensure accurate and timely discharge.
  • Participate in the Length of Stay (LOS) report weekly meetings and other relevant meetings to drive improvement.
