Community Care Specialist

We are seeking dedicated individuals who can provide person-centered care and support to adults with autism.
About the Job:
This role involves working as part of a multidisciplinary team to provide support and guidance to individuals in their community, helping them to achieve their goals and aspirations.
You will have the opportunity to work closely with colleagues and other stakeholders to develop and implement individualized support plans that promote independence, dignity, and well-being.
Requirements:
- Ability to work flexibly, including dayshift and nightshift positions
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of health and social care policies and procedures
- Experience working with adults with autism or similar conditions (preferred)
Salary and Benefits:
The estimated salary for this role is £20,000 - £25,000 per annum, depending on experience and qualifications.
