About the Role

This is an exciting opportunity to join our team as a Band 7 Senior Community Mental Health Practitioner, working closely with patients and families to provide compassionate and effective care. You will be part of a multidisciplinary team, collaborating with GPs, social workers, and other healthcare professionals to deliver person-centered care.

Key Responsibilities

Your key responsibilities will include:

  • Conducting thorough assessments and creating individualized care plans
  • Delivering evidence-based interventions and therapies to promote patient recovery and well-being
  • Liaising with external agencies and partners to ensure coordinated care

Person Specification

To succeed in this role, you will require:

  • NMC registration as a nurse or allied health professional
  • Proven experience in a mental health setting, preferably with a focus on community care
  • Minimum 3 years post-qualification experience, with expertise in mental health theories and models
