The Role

As a Medical Social Worker, you will play a critical role in supporting our patients and their families through:

  1. Comprehensive assessments and care planning to address social, emotional, and environmental factors affecting patient well-being.
  2. Collaboration with the interdisciplinary team to ensure seamless coordination of care.
  3. Connection to community resources and support services to enhance patient outcomes.

Your Expertise

We seek a skilled Medical Social Worker with:

  1. A Master's degree in social work from an accredited school of social work.
  2. Proven experience in medical social work, preferably in home health settings.
  3. Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare professionals.
