About the Role
The Young Adults and Independent Living Team at Central Bedfordshire Council is seeking a dedicated professional to support vulnerable individuals with Learning Disabilities, Autistic Spectrum Conditions, or Physical Disabilities.
As a key member of the Adults Team, you will work closely with young people aged 16+ to build strong relationships, assess, review, and develop support plans that cater to their unique needs.
Key responsibilities include:
- Collaborating with local communities, professionals, and employers to ensure individuals have access to fulfilling lives and achieve their aspirations.
- Assessing and reviewing support plans to ensure they meet the evolving needs of service users.
- Providing proactive and approachable support to individuals, their families, and carers.
- Working closely with health, housing, and education partners to ensure seamless service delivery.
The ideal candidate will possess a strong understanding of the SEND reforms, the Care Act 2014, and sound care management skills. They will be tenacious, proactive, and diplomatic, with the ability to build professional relationships and navigate complex systems.
