Job Overview

We are seeking a skilled and compassionate Children's Care Professional to join our team in Ashton-under-Lyne. As a key member of our leaving care team, you will play a vital role in supporting young people through their transition to independence.

About the Role

This part-time position offers 21 hours per week over three days and is an excellent opportunity for someone looking for a rewarding career in youth support. You will be working with young people who have experienced care and are navigating the complexities of adulthood.

Key Responsibilities
  1. Provide personalized support to young people within their home or placement settings.
  2. Attend and plan reviews and home visits to ensure seamless transitions and effective planning.
  3. Maintain regular face-to-face contact with young people referred to the project, fostering engagement and empowerment.
  4. Conduct thorough support needs analyses and develop individual plans to address future aims and objectives.
  5. Implement review recommendations and pathway plans, promoting positive outcomes.
  6. Collaborate with young people to achieve their goals, focusing on education, training, employment, and personal development.
  7. Offer guidance on accessing positive activities, housing, health support, and other essential services.
  8. Build trusting relationships with young people, enhancing their self-esteem, confidence, knowledge, and resilience.
  9. Respond promptly to any child protection/Adult at Risk concerns, upholding safeguarding standards.
  10. Liaise with other agencies on behalf of young people, ensuring cohesive support networks.
  11. Maintain accurate records of work completed and attend relevant meetings, reviews, and forums.
