About the Role

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team at Giffard Drive Surgery. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality nursing services to our patients with long-term conditions.

Main Responsibilities

  • To develop and implement effective care management programs for patients with chronic conditions, including diabetes, hypertension, and respiratory diseases.
  • To work closely with the practice management team to ensure the delivery of clinical policy and procedure.
  • To provide guidance and support to patients on the use of prescribed and over-the-counter medications.
  • To coordinate the provision of travel medicine services and oversee the provision of chronic disease clinics.
  • To maintain accurate clinical records and ensure the appropriate maintenance and use of chronic disease registers.

Requirements

  • Registered Nurse with the Nursing and Midwifery Council.
  • Experience in chronic disease management, including diabetes, hypertension, and respiratory conditions.
  • Previous Practice Nurse experience.
  • Qualified smear taker.
  • Imms/Vacs trained, including childhood imms.
