About the Role

We are seeking a highly skilled and experienced Practice Nurse to join our team at Giffard Drive Surgery. As a Practice Nurse, you will play a vital role in the delivery of high-quality patient care, working closely with our multidisciplinary team to ensure the effective management of patients with long-term conditions.

Main Responsibilities

  • To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice.
  • To work as a senior member of the practice multidisciplinary team, ensuring nursing services are delivered effectively to the entitled patient population.
  • To be responsible for a number of clinical areas, including infection prevention and control, health promotion, chronic disease management, and well woman and well man clinics.
  • To actively support the practice management team in the reviewing and delivery of clinical policy and procedure.

Requirements

  • Registered Nurse with the Nursing and Midwifery Council.
  • Experience in chronic disease management, including diabetes, hypertension, and respiratory conditions.
  • Previous Practice Nurse experience.
  • Qualified smear taker.
  • Imms/Vacs trained, including childhood imms.
