Main Responsibilities- Use a patient-centric approach to complete nursing assessments and formulate discharge plans.
- Work collaboratively with Specialist Nurses and Physiotherapists to provide comprehensive care.
- Develop and implement discharge plans to ensure smooth transitions for patients.
- Contribute to the development of the Hospital Discharge Team and participate in service improvement initiatives.
- Current registration with the Nursing and Midwifery Council (NMC).
